GENERAL INSTRUCTIONS

1. Carefully read the instructions before completing the questionnaire. Note that submission of false information will lead to automatic disqualification.
2. Responses to the prequalification questionnaire must be in accordance with the requirements for information in the documents.
3. The supplier should prepare a table of contents as given below on their letterhead, proper document separators, and section labels to be done.
4. Submission of documents: The application for prequalification must be submitted via sealed envelope at the given address and must mention the applied category.
5. The documents should be signed by the authorized representative of the firm, stamped, and submitted with relevant supporting documents such as references, certificates, tax returns and any other information that the applicant wishes to be considered.
6. Applications may be modified or withdrawn in writing, prior to the closing time specified in this Document. Applications shall not be modified or withdrawn after the deadline.
7. The applicant shall bear all costs associated with the preparation and submission of the Application and RIZ Consulting will not in any case be responsible and liable for the costs incurred.
8. All information given in writing to or verbally shared with the contractor(s) in connection with this pre-qualification is to be treated as strictly confidential. The contractor shall not share or invoke such information with any third party without the prior written approval of RIZ Consulting. This obligation shall continue throughout the contract period whether the contractor is successful.
9. RIZ Consulting reserves the right to accept, reject or cancel the pre-qualification process without assigning any reasons at any time, whole or in part.

Preliminary Examination / Mandatory Requirements

Successful / Unsuccessful: If Vendor does not qualify the preliminary examination stage that will not consider in further evaluation.

1. Must have Valid NTN (Proof of being Active Taxpayer), attach tax certificate.
2. For Computers, vendor must provide proof of being an authorized reseller, attach valid certificate.
3. For medical supplies must provide proof of authorized reseller/distribution, attach valid certificate.
4. List of clients.
5. For Construction firms (Proof of PEC, SECP)
6. For tour operators & travel agents (Proof of DTS & IATA certificate)

IMPORTANT NOTE: -

All the applicants, please note that all the documents/statements submitted by a Firm/Company are for its pre-qualification purpose only.

Any document/statement provided if proved false, misstated, concocted, or incorrect at any time during or after Pre-qualification will result into permanent disqualification from this process.
